A Logic Model is a strategic map that illustrates how a specific, concrete activity is connected step-by-step to a large, long-term social change.

Each level of the logic model is assigned a specific numerical code (e.g., 1000, 1100, 1200, 1300, 1220, 1222). These numbers are not random administrative placeholders; they indicate exactly where a piece of work lives within the broader strategy and what higher-level outcome it is meant to feed into.

THE FOUR LEVELS OF THE LOGIC MODEL

1. OUTPUTS

2. IMMEDIATE OUTCOMES

3. INTERMEDIATE OUTCOMES

4. ULTIMATE OUTCOME (1000)
